1. Complete and submit the application form to the AFCM office.
2. Ensure pastoral recommendation form is received by the AFCM office (link to form: https://afcm.breezechms.com/form/0ee5eb7690).
3. Your AFCM Regional Director may schedule an interview as part of the process.
4. Await notification by mail regarding the decision on your application.
1. Membership expires annually on the first day of January. The renewal fee is $50.00.
2. Renewal forms need to be completed before December 31 to avoid a late fee.
4. A late fee of $25.00 will be applied to all renewals received online or postmarked after December 31st and will not be processed until the late fee is paid.
5. Those whose applications are approved in the last quarter of the year will not pay a renewal fee for the following year.
6. Those whose renewals are not received before March 1 will be placed on the inactive list and required to return their certificates.
