Renewal/Next Module Request Instructions

1. Requesting the Next Module

After completing a module, schools may choose to continue with the next module in sequence.

Each new module costs $550.

A Next Module Request Form must be submitted along with full payment.

A separate application is not required for continuing within the same level (for example, from Module 1 to Module 2 or from Module 2 to Module 3).

Once Level One (Modules 1–3) is completed, schools must submit a new application to begin Level Two (Modules 4–6).

2. Renewing Previously Completed Modules

Schools may renew and re-show previously completed modules at any time.

Renewal fee is $250 per module.

A Renewal Request Form must be submitted along with full payment.

Schools may operate multiple classes simultaneously as long as each module being shown has been renewed for the current year.

If a school chooses not to continue, all recorded materials must be returned to AFCM ITC within 30 days of closure at the affiliate school’s risk and expense.

If your school was originally issued on DVD and you would prefer the convenience of a flash drive, you may upgrade your materials for a fee of $50.

We understand that by renewing or continuing, we agree to abide by the original policies and licensing agreement signed when our school was first approved. We agree to send a list of enrolled students to AFCMITC within 30 days after our new class begins. We also agree to submit final grades for each student at the end of each module.
